Clinic Information

Laurie Houghland RN
220-4079

Medication Policy

If medication is necessary during school hours it must be brought to the clinic by a parent or guardian in the original container with the appropriate label. A permission form must also be filled out at that time.  Non-prescription medications such as Tylenol and Motrin may also be sent in the original containers.  Parents must complete and sign one permission form per medication.  (See page 11 of the Student Handbook.)

Self-Administering Asthma Medicine

Absences

Please report all absences to the office at 220-4078 or 220-4080.  After hours voice mail is available.  The parent/guardian is responsible for providing the school a note within 24 hours of the absence indicating the reason for the absence.  (See page 3 of the Student Handbook.)  If any of the following signs and symptoms are present in you child, we ask that the student remain at home:  fever of greater than 100 degrees or more, vomiting, repeated diarrhea, inflamed or draining eyes or ears, persistent cough/sore throat, skin eruptions/draining sores.  Working together the spread of illness can be minimized.  (See page 11 of the Student Handbook.)

Health Update Forms

If you haven’t already filled out the “orange” Health Update Form, please do so immediately as we may need to contact a parent/guardian in the case of an emergency.  If you are not sure if you have filled out the new form feel free to call me at the clinic at 220-4079.
