The proposal for the science fair
project includes:
- Sponsor checklist: Fill out your name and project title.
- Student checklist (1A): Fill out in its entirety.
- Approval Form (1B): Fill out, sign and date. Parent
must sign and date.
- Research Plan: follow guidelines on Research Plan
instructions. Should include: question, hypothesis/problem,
detailed description of method/procedures, data analysis procedures,
and bibliography.
If you are using human subjects, vertebrate animals, potentially
hazardous biological agents, or hazardous chemical activitites and
devices there are special forms for you.
