Clinic Information

The clinic provides a valuable service for students and staff who are ill, have health problems, or need emergency care. The School Nurse is on duty during the school day to take care of individuals who are sick.
What are the clinic procedures?
1) A student will be admitted to the clinic only when a teacher or administrator gives the student a clinic pass or signs their agenda - or in cases of emergency. Teachers should be sure that the students name, the date, and the time are on clinic passes. Students are to use their agendas for clinic passes.
2) Students are to report directly to the clinic when authorized to do so. Upon entering the clinic, students will sign their name in the Clinic Log. The time that the student enters and departs the clinic will be recorded. This procedure provides a record of the number of clinic visits, the frequency of clinic visits, and the length of time a student is in the clinic.
3) Students are not to come to the clinic between classes. If a student does come at this time, the student will be sent to the next period class for a pass to return to the clinic. Please remind students. . . STUDENTS MUST HAVE A PASS TO GO TO THE CLINIC. NO STUDENT IS TO SEND HIMSELF/HERSELF TO THE CLINIC.
4) All students who have to be sent home ill through the clinic will have their names, departure time, and the person with whom they left recorded in the Clinic Dismissal Log. In the case of early dismissals through the clinic, the parent/parent designee will be contacted. The Attendance Office will be notified of early dismissals through the clinic. Clinic dismissals can be found on the daily attendance sheet as provided by attendance.
5) When students are ready to return to class from the clinic, the nurse will sign the clinic pass and indicate the time that the student leaves the clinic.
6) There will be times when the nurse needs to see students to administer eye exams, hearing tests, etc. Students may be called to the clinic via the PA system or by appointment slips that will be placed in your mail box.
7) All medications must be kept in the clinic in the original container, labeled with the students name, the physicians name, the name of the medication, the dosage, and the time the medication is to be given. All medication must have a parental permission form completed and on file in the clinic. The medication must be taken as scheduled in the clinic.
8) Teachers should use their own discretion in giving students clinic passes. Until a student abuses the clinic pass, he/she should not be denied such a pass.
9) If an emergency occurs in the classroom, send a reliable student to get the nurse or use your classroom phone. If possible, provide a brief assessment of the situation. Please identify the student with the emergency.
10) A confidential list of students with medical conditions is available in the clinic for your review.
