As the school nurse at GBES, I am proud to be part of a wonderful team that supports your child’s success and well being this year. As I work with you and your child, I will need your assistance in preparing for the possibility that your child may become ill, need medication, or have an accident during school hours. The following information will guide you through any questions you may have.
1. A new student entering GBES (including Kindergarten students) must have documentation of a complete comprehensive physical. This physical should be on a blue Virginia School Entrance Health form. This physical should include a urinalysis and a screening for iron-deficiency anemia.
2. New students coming from outside of Virginia will also need a complete comprehensive physical as well. This physical must meet all of the requirements on the Virginia School Entrance Health form. 3. Physicals must be performed within 12 months prior to the child’s first day of school.
4. Documentation of completed immunizations of each required vaccine MUST BE MET PRIOR TO ENTRY TO SCHOOL. Click Here for Immunization Schedule
Helpful Clinic Information Attendance:
Please report all absences to the clinic at 890-1006 (voice mail is available 24 hours/day) This is a safety precaution and we will be calling you at home to verify student absences in compliance with Virginia State laws if we don’t hear from you.
Illness:
After a child is sick, please do not send them back until they have gone 24 hours without a fever, vomiting, diarrhea, or muscle fatigue. Remember the following: “If your child is sick today, then tomorrow they shouldn’t return to school or play”.
Medications:
If medication is necessary during the school day, it must be brought to the clinic by a parent or guardian in the original container with the appropriate label. A signed permission form must be completed.
Cough Drops at School:
If you child needs to use cough drops during school, the parent needs to send the product in and have it kept in the clinic. I need a parent’s written request and the understanding that the responsibility of consumption of the cough drop belongs to the parent and child - - not the teacher, nurse or school. Cough drops can be a choking hazard and the safety of our students is our top priority.
P.E. Excuses:
Illness, and injury can result in the need for limited or restricted PE activities. This requires a written note from a parent or guardian as per the Student Handbook states on page 23. “The only acceptable excuse from physical education for more than three consecutive days is a doctor’s excuse.” This is especially true with sprains and breaks. We need specific instructions from the physician to insure your child’s safety.
Health Cards and Emergency Information:
Please complete the orange Annual Health Update cards promptly so we are aware of current phone numbers, health status and medications. We must also have an emergency number for each student in case of an emergency. Please include your cell phones and pager numbers.
Forms for Students with Special Needs
If your child has food allergies, please have your doctor complete the “special dietary needs” form and return to the clinic ASAP. Click Here for Special Dietary Needs Form For Students with food allergies that require an Epi Pen, we have a special form “Food Allergy Action Plan”. We need this filled out and returned to the clinic with your child’s photo. Click Here for the Food Allergy Action Plan Form

What To Do If My Child Get’s Head Lice
Head lice are a problem in communities nationwide. Anyone can get head lice and it is not a reflection of cleanliness or socioeconomic status. Lice actually prefer clean scalps and spread simply by close contact with an affected child, their clothing, combs, brushes and hair accessories.
Once all steps are complete, children may return to school by:
1. Returning to the nurse’s office with a parent or guardian to have their head’s rechecked for lice or nits. (ALL NITS MUST BE REMOVED BEFORE THEY CAN RETURN TO SCHOOL). This is a York County School Division policy.
2. If possible, bring proof of treatment.
3. Retreat 7 – 10 days later and continue to monitor hair daily for nits and lice.
